Brand Name
Tucker Sling and Foam Wedge
Manufacturer
Tucker Designs, LTD
Model Name
Tucker Sling and Foam Wedge Combination
Product Description
This product is a foam-based sleep wedge combined with a fabric sling that secures the infant in an inclined position. Marketed as a solution for babies with reflux or breathing issues, the wedge is placed inside a crib or bassinet, and the sling wraps around the baby’s torso and shoulders.
Case Details
On June 22, 2024, a mother discovered her 5-month-old daughter unresponsive in her crib. The infant had been secured to a foam wedge using a fabric sling, a combination product from Tucker Designs, marketed for supporting babies in an elevated sleep position.
According to the incident report, the product appeared to be used exactly as intended, and was reportedly provided by Seattle Children’s Hospital following the child’s birth. Despite this, the child died during sleep, and investigators are awaiting additional information on possible injuries and cause of death.
Injury Brief
Inclined sleep positioners — including foam wedges, nests, or sling-based systems — have been under growing scrutiny from pediatric experts, the American Academy of Pediatrics (AAP), and federal safety agencies like the Consumer Product Safety Commission (CPSC).
These products have been linked to:
- Asphyxiation risks due to chin-to-chest positioning
- Restricted airflow when babies roll or slump
- False reassurance for parents that inclined devices are medically approved or safe for sleep
Despite mounting evidence and recalls involving similar products, items like the Tucker Sling and Foam Wedge Combination continue to be marketed online and distributed through non-traditional retail channels.
Retailers
The mattress tag on the unit in question had been removed, leaving no identifiable serial number, SKU, or UPC. All available product information was obtained through the product’s website:

This lack of permanent labeling raises troubling issues:
- Consumers may be unaware of safety updates, recalls, or liability claims
- Used or handed-down products may circulate without updated safety guidance
- Manufacturers can evade accountability without traceable serial data
Liability
Although no defect has yet been confirmed, manufacturers of infant sleep products are responsible for:
- Ensuring their products comply with federal safety standards
- Avoiding marketing products for sleep use if not approved for it
- Including clear and permanent warnings about usage limitations
- Preventing false associations with hospitals or medical endorsements
